INFORMATION FOR PATIENTS REGARDING CORONAVIRUS (COVID-19)

We are commited to keeping our most vulnerable patients and staff as safe as possible, therefore we have suspended all routine face-to-face GP appointment bookings. Patients who had an appointment booked will be telephoned by a GP at the time of the original appointment. If you have not spoken to a doctor please do not attend the surgery.

In line with the latest advice, if you have a high temperature or new, continuous cough you should self-isolate for a period of 7 days. There is no need to contact either the surgery or NHS111.

Online 'Pre-Registration' With The Practice

PreRegistrationIf you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us.

Please note that you must send us both forms.

After sending both forms you must come to the surgery to sign them and for us to confirm the details. You will not be a registered patient until this has been completed.

Pre-registration Form

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.

Note that by sending the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.


Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice.
